Our Council commissioned Passport To Health programme offers support and guidance to individuals living with health issues, helping them improve their health and well-being.
Staying active is the miracle cure and is crucial for a long and happy life! Our qualified and friendly staff will help you tailor a safe and effective activity programme to help you achieve this.
A health professional can refer those suffering from hypertension, obesity, diabetes, or mental health issues to the scheme. Please get in touch with your GP or health professional for further information. Please note that referrals are submitted to a central database managed by a BANES Council-commissioned service provider, where eligible referrals will be contacted once their referral has been accepted.

CVD Risk criteria
Inactive (< 60 mins p/w)
BMI >30 (or >27 for BME patients)
Family history of CHD
High but stable Blood Pressure <180/100
High cholesterol
Type 2 diabetes
Exercise for Depression criteria
Inactive (< 60 mins p/w)
Low mood/stress
Mild/moderate depression/PND
